- Arteries carry oxygen-rich blood from the heart to other organs in the body and veins carry oxygen-poor blood back to the heart.
Varicose veins are swollen veins that can be seen through the skin. They are often blue, bulging and tortuous. If left untreated, varicose veins will worsen over time.
🔹 Stage C2–C3: Disease Begins to Progress
The veins become visibly enlarged, bulging, and twisted, with a diameter over 3mm. Swelling around the ankles may begin.
  • Symptoms: Leg pain, heaviness, numbness, night cramps, mild swelling, visible bluish-purple veins on the skin
  • Risk: This is the best time to treat the condition effectively. Without prompt intervention, it can progress rapidly and become difficult to control – potentially leading to phlebitis or blood clots (venous thrombosis)
🔹 Stage C4: Blood Stasis – Skin Damage
Veins become significantly enlarged, like chopsticks. Blood begins to pool excessively.
  • Symptoms: Darkened skin, pigmentation, hardened or leathery skin, skin dimples when pressed
  • Risk: Prolonged blood pooling causes fluid leakage under the skin → leading to inflammation and patchy skin necrosis
🔹 Stage C5–C6: Ulceration – Tissue Necrosis – Risk of Stroke
  • Symptoms: Skin ulcers, persistent dark patches, tissue death, vascular blockage, blood clots forming in the veins
  • Risk: Very high – includes vein rupture, pulmonary embolism, stroke, or limb amputation if left untreated
The danger of varicose veins is that they can easily cause
Thrombosis complications: due to blood stagnation in the blood vessel wall, which can cause local blockage, creating ulcers, infections, and in many cases necrosis, requiring amputation of the leg.
Not stopping there, these blood clots can also flow through the bloodstream, and wherever they go, they will cause blockage there. Specifically, thrombosis is blocked in:

  • Cerebral blood vessels causing cerebral ischemia, stroke.
  • Coronary arteries causing myocardial infarction.
  • Pulmonary arteries causing pulmonary embolism, respiratory failure
The veins begin to dilate slightly, with a diameter of less than 3mm. Common areas affected include the inner ankle, calf, and inner thigh.
🔹 Stage 4 venous insufficiency:
Darkening of the skin in the lower leg accompanied by edema. Note the indentation after pressing a finger on the dorsal side of the right foot, which is an indication of leg edema. Although this patient's lower legs do not have varicose veins, Doppler ultrasound of the blood vessels shows reflux in both superficial and deep veins in both legs, more pronounced in the right leg.

skin pigmentation changes accompanied by healed ulcers.

skin pigmentation changes accompanied by active, non-healing ulcers.

Dilated spider veins below the inner ankle. The dilated veins are less than 1 mm in diameter.
🔹 Stage 2 venous insufficiency:
Enlarged, tortuous superficial veins under the skin, with a diameter greater than 3 mm.

Edema in the feet and ankles. Edema can be caused by various factors such as heart failure, kidney failure, cirrhosis, malnutrition, etc. Venous edema often appears after prolonged standing, is usually not noticeable in the early morning, and is more pronounced in the afternoon, usually only affecting the legs and not other areas of the body.
